Medical / Health - Administration Relocation Assistance Available - Possible for ideal candidate Job Highlights The Director of Case Management and Social Work is accountable for ensuring effective patient throughput and efficiency in processes related to providing patients with the right care at the right time. This role provides leadership, direction, and guidance to designated department(s) and programs; oversees operations and identifies key barriers/core problems requiring the application of problem-solving skills in order to deal with complex situations. This position will have oversight of case management and care continuum in our Meridian, Nampa, and Fruitland population health areas. Responsibilities Leads and oversees a multi-functional clinical team responsible for driving the highest quality care coordination, patient experiences and outcomes. Functional areas of accountability may include Case Management, Social and Behavioral Services, Rehabilitation Services and/or Home Care/Hospice. Works with the Post-Acute Care Center of Excellence (CoE) to develops and implements clinical standards of practice, clinical supervision policies, procedures and practice protocols to ensure excellent patient care and quality outcomes through coordination of care across the continuum. Partners with Center of Excellence (CoE) and area leadership to implement and execute on focused strategies, including patient throughput and placement; cost and utilization optimization; care coordination; and quality outcomes. Manages planning with initiation of departmental goal setting and tracking on a regular basis. Serves as a subject matter expert on benefits and any limitations therein, and provides guidance on how to address member/patient needs when benefits are limited. Consults on the impact in the continuum for all initiatives that result in a shorter hospital length of stay. Partners with leaders to address patient placement barriers. Leads efforts to connect with community resources and within the local post-acute care industry in order to develop relationships and to stay connected to industry changes and new regulations. Provides leadership and oversight for effective fiscal management, performance improvement activities, and compliance with all relevant policies/procedures and regulations. Actively participates in the budget and goal setting process. Direct human resource initiatives and talent management efforts for designated areas. Provides leadership, guidance, development and continuing education to staff. Selects, develops, coaches, mentors, and assesses performance of staff. Interacts with operating units and area leadership teams to provide consultation, influence decisions and identify and resolve critical issues. Develop positive working relationships with all internal and external stakeholders and customers. Other duties and responsibilities as assigned.
Qualifications Education:
Bachelor's Degree in Health Services, Business Administration, Public Health, Nursing, Social Work or related field.
Experience:
10 years' experience in clinical and managerial roles in a multi-faceted health care system and/or multi-service provider setting
Salary Range:
$120K -- $200K
Minimum Qualification
Allied Health Professionals, Healthcare ManagementEstimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
