Copper Falls is recruiting experienced and compassionate hospice professionals and is currently accepting applications for a PRN Licensed Masters Social Worker position.
General
Responsibilities:
Help families navigate through end of life care Obtains community resources for patient and their family Completes psychosocial assessments of the patients and their families Assists in the interventions and goals related to the patients care plan Advocate for patients end-of-life wishes Complete detailed charts to summarize interactions with patients Full job description available upon request Ideal Candidate Skills/Knowledge:
